I understand that there are risks involved in such participation and that it is the responsibility of each participant to engage in only those activities for which he/she is comfortable. I accept the responsibilities for all risks involved with my child participating in physical activities. I understand that I have been encouraged to check with my doctor to ensure that my child is able to perform these activities. I certify that my child is physically and mentally fit and able to participate in these activities and I authorize Friends of Bishop Ryan Staff to administer emergency medical treatment if required.

I, the undersigned agree to permit Friends of Bishop Ryan Summer Sport Camp and any person authorized by it/them, to take and produce photographs, digital images and other visual or audio reproductions of myself or a member of my family captured during regular and special Summer Sport Camp activities.

Photo Release: During the course of our camps and activities, we take photographs of our participants to share positive vibe and updates. Incidentally, some photographs may capture your child's participation, directly or indirectly. These photos may be published through our website, social media pages, news bulletins, billboards, and ads. With this Photography release, we seek your consent in allowing us to publish photos which may involve your child to said platforms. Please provide your response by selecting your choice below.
